Additional Information Request

Because of current tax laws in the USA, when your earnings hit the threshold of $600, we are legally obligated to issue a 1099 to you and the Internal Revenue Service.  When you earn that amount, we will not process your payments until you submit more information so that we can remain in compliance with the reporting requirements.  You can submit this form at any time and your earnings will continue to accrue without penalty until we receive the necessary information.
